Cholecystitis: Symptoms, Causes, and Surgical Treatment

Cholecystitis is the medical term for inflammation of the gallbladder—a small, pear-shaped organ located beneath the liver on the right side of the abdomen. Its primary role is to store bile, which helps digest fats. When bile flow is obstructed, it causes the gallbladder to become swollen and inflamed. If left untreated, it can lead to severe complications, such as a gallbladder rupture, which can be life-threatening.

Common Symptoms: How to Identify Cholecystitis

Symptoms often occur suddenly, particularly after consuming a heavy or high-fat meal:

  • Severe Abdominal Pain: Intense pain in the upper right or center of the abdomen, which may radiate to the right shoulder or back.
  • Tenderness: Significant pain when the right side of the abdomen is touched or pressed.
  • Associated Symptoms: High fever, nausea, vomiting, bloating, and loss of appetite.
  • Warning Signs: Yellowing of the skin and eyes (Jaundice) or clay-colored stools.
    Note: If you experience severe abdominal pain lasting for several hours accompanied by a fever, seek medical attention immediately to prevent complications.

What Causes Gallbladder Inflammation?

  1. Gallstones (Calculous Cholecystitis): The most common cause (95%). Gallstones block the cystic duct, causing bile to become trapped, leading to inflammation and potential bacterial infection.
  2. Other Causes (Acalculous Cholecystitis): Less common but often more severe. This can be caused by tumors in the liver or pancreas, blood infections (Sepsis), or complications from chronic conditions like diabetes.

Diagnosis and Treatment

Doctors typically use blood tests to check for infection and liver function, combined with Abdominal Ultrasound or a CT Scan to visualize the gallbladder’s size and the presence of stones.

  • Treatment Pathways:
    Initial Management: Patients are usually fasted (NPO) to rest the gallbladder. Intravenous (IV) fluids, pain relief, and antibiotics are provided to stabilize the condition.
  • Surgical Treatment (Cholecystectomy): Removing the gallbladder is the standard treatment to prevent recurrence.
      • Laparoscopic Surgery: The most common technique involving small incisions. It results in less pain and a much faster recovery.
      • Open Surgery: Required in severe cases or when complex complications are present.

Dietary Guide: Foods to Avoid

Managing your diet is crucial to reducing the workload on your gallbladder:

  • High-Fat Foods: Fried foods, fatty meats, sausages, and bacon.
  • Full-Fat Dairy: Butter, cheese, and heavy cream.
  • Sugary Foods: Cakes, ice cream, and sugary drinks (sugar can trigger stone formation).
  • Stimulants: Alcohol and excessive caffeine, which can cause the gallbladder to contract painfully.

Prevention and Lifestyle Tips

  1. Maintain a Healthy Weight: Obesity increases cholesterol in bile, leading to stones.
  2. Lose Weight Gradually: Aim for 0.5–1 kg per week. Rapid weight loss can actually trigger gallstone formation.
  3. High-Fiber Diet: Focus on vegetables, fruits, and whole grains.
  4. Regular Exercise: Helps lower cholesterol and improves overall digestive health.
